Abstract

Pharmaceutical care is the responsible provision of drug therapy for the purpose of achieving definite outcomes that improve a patient’s quality of life. Ward pharmacy is one way to realizing pharmaceutical care. This research’s aim was to study potential drug interactions in geriatric patient hospitalized at Dahlia ward Prof. Dr. Margono Soekarjo hospital, before and after ward pharmacy. The research used before and after study method, comparing potential drug interactions before and after ward pharmacy from March - June 2006. The different significance of drug interactions before and after ward pharmacy tested with Mann-Whitney test. Software which used in statistical analysis was SPSS (Statistical Product Social Science for) version 11.The study showed that total potential drug interaction before and after ward pharmacy were 3,69% and 5,12%. Drug interactions with significance rating 1, 2, and 3 before ward pharmacy were 0.08 ; 0.16 and 0.04 respectively ; while after ward pharmacy were 0.33 ; 0.14 and 0.09 respectively. The statistical analysis indicate that there was no a significance difference of drug interactions before and  after ward pharmacy (P> 0,05). The result was used to improve the system of pharmaceutical care in RSUD Prof. Dr Margono Soekarjo Purwokerto.Key words: ward pharmacy, geriatric patient, potential drug interaction

Abstract

In continuing of chemical study research on the parts of the fruits of Mahkota dewa, we have done isolated one antioxidant compound benzophenon glycoside from n-butanol extract. Isolation and purification by column chromatography (SiO2, chloroform-methanol) and determination of chemical structure based on interpretation spectra of ultraviolet (UV), infrared (IR) and nuclear magnetic resonance 1 dimension (1H & 13C NMR), 2 dimension (1H-1H COSY, 13C-1H COSY, HMBC). Based on spectrroscopic data, the compound was identified as 6,4’,-dihidroksi-4- metoksibenzofenon-2-O-α-D glucopyranoside.Key words: Mahkota dewa, Phaleria macrocarpa, Thymelaceae, Indonesian medicinal plant, benzophenone glycoside

Abstract

Carica papaya L. leave was known containing Ribosome-inactivating proteins (RIPs), which demonstrated to have an in vitro cytotoxic effect on cancer cell lines. This researh examined the effect of protein fraction containing RIPs isolated from Carica papaya L. on the expressions of p53 and Bcl-2, regulator proteins on apoptosis process, in breast cancer cell lines (T47D).RIPs from Carica papaya L. leaves were isolated by amonium sulfat precipitation. This fraction was analyzed by the activity of cleaving supercoiled double stranded DNA, in order to identify the presence of RIP. The active fraction was then tested of the citotoxic activity on breast cancer cell lines followed by analysing the expressions of p53 and bcl2 using immunohistochemistry technique.The results indicated that the protein fraction possessed cytotoxic activity in breast cancer cell line with the IC50 of 2.8 mg/mL. The expression level of p53 was increased by 59.4%, while Bcl-2 protein was \ decreased by 63%. Abstract

The increasing number of drugs available and the increasing use of multidrug regimens which enhance the possibility for drug interactions. The aim of the study was to determine the frequency of drug-drug interaction and drug-food interaction both in outpatients and hospitalized patients. The research was also to determine the pattern of drug interaction mechanism and drugs which often show interaction.The Research type was descriptive. The data were conducted by retrospective, simple random sampling, taken from 90 samples of geriatrics patient hospitalized medical record in Interne medicine during June- September 2003, 120 samples of outpatient prescription took care from Haspita Farma Pharmacies during July-December 2003. Evaluation of the data was carried out descriptively.The result of the study showed that drug interaction occurred in 59% cases of hospitalized patient and 69% cases of outpatient. It was identified the existence of 125 drug interaction in hospitalized patient consists of 48 drug-drug interaction and 77 drug-food interaction. The pattern of drug interaction mechanism was pharmacokinetics pattern of drug interaction mechanism 36%, pharmacodinamic 16 % and unknown 48 %. The drug which often have interaction were furosemid, captopril, aspirin, and ceftriakson. In outpatient, It was identified 128 drug interactions consist of 47 drug-drug interactions case and 81 drug-food interaction. The mechanism pattern was pharmacokinetic 72%, pharmacodinamic 19% and unknown 11 %.The drug which often had interaction was phenitoin, phenobarbital, isoniasid, and rifampicin.Key words : drug interaction, Dr. Sardjito Hospital, Geriatric

Abstract

Drug interaction can happen when two or more drugs are given together. This research was aimed to observe the effect of PGV-0 pretreatment to paracetamol pharmacokinetic profile in male rats.The study was conducted employing a one-way randomized completely design, using male Wistar rats weight 150 g (±10%). The animals were divided into three groups (5 rats for each group). Group I (control) was given a single oral paracetamol 150 mg/kg BW. The other groups II dan III were given a single oral PGV-0 20 and 40 mg/kg BW one hour before treatment with paracetamol respectively. After all rats were pretreated, serial blood and urine samples were withdrawn and were analysed using HPLC for unchanged paracetamol. Pharmacokinetic parameters of paracetamol i.e. Cmax, tmax, AUC, K, t1/2, Vdss/F, Cl/F, Aecum, dan %fe were determined based on concentration to time data in the blood and urine. The paracetamol pharmacokinetic parameters were analyzed by one-way analysis of varians (ANOVA) using 95% confidence interval. And the difference between groups were analyzed using Tukey-test method. The results showed that the pharmacokinetic parameters of paracetamol in the animals prefed with PGV-0 20 and 40 mg/kg BW did not change significantly (P > 0.05).Key words : pharmacokinetics, interaction, paracetamol, Pentagamavunon-0

Abstract

It is known that piper leaves (sirih) from Piper betle Linn has been used traditionally as medicine, for example for mouthwash and wound curing. The usage of hand gels antiseptic preparation was getting increased in the current time as the incidence of desease caused by virus and bacteria like SARS appears.This research was aimed to develop preparation of hand gels from piper leaves extract obtained from extraction with hot water. The formulation of gels were made using 5%, 10%, 15%, 20% and 25% of piper leaves extract in Carbopol gel base. The evaluation of antiseptics activity was carried out using the Replica method. A series of test was done by pouring the gels at hand palm and slightly tap the thumb fingerprint to nutrient agar media. The media was incubated at 37°C for 24 hours,and the number of colonies of microorganism was counted. The antiseptic activity of the gels also compared with two hand gels antiseptic preparation containing ethanol and triclosan as antiseptic agentThe result showed that the piper leaves extract gels had a transparent pale yellow performance. The gel preparation with concentration of 15% extract had activity to reduce the microorganism of skin normal flora up to 50%. Yet the 25% concentration of piper leave extract could eliminate all the microorganism. It was also known that the 15% piper leaves extract gel had no significance different with the ethanol preparation, since the 20% and 25% piper leaves extract gels had similar antiseptic activity to the triclosan preparation.Key words: piper leaves extract, gel, antiseptic activity, replica methods

Abstract

Potassium PGV-0 was a salt form of PGV-0 which has been developed to be a new medicine. The salt form was relatively water soluble and therefore was is expected to have better bioavailability and efficacy than the parent compound.The research aims were to investigate the pharmacokinetics profiles of PGV-0 after the administration of its potassium salt per orally in male rats Wistar The treadment was done by using a completely randomized design to explore the profiles of PGV-0 in blood, its distribution into primary organs : liver, kidneys, lungs and small intestine, and also its elimination into urine and feces in the animals. Potassium PGV-0 was given single doses per orally at 40 and 80 mg/kg BW. The analytical assay for PGV-0 was conducted by an HPLC method with a UV-Vis detector.The results shown that according to per oral administration of K-PGV-0 to the rats, there was no PGV-0 found in the blood monitored up to 360 minutes. The compound was not found in the liver nor kidneys, and only traces (<5% of dose) were found in the lungs or small intestine. Traces amounts of PGV-0 were also found in urine or feces. The extremely rapid disappearance of the compound from the blood may not be due to its rapid distribution into the organs but more likely due to its fast biotransformation in the blood or in the liver. This assumption is also supported by the facts that PGV-0 found in the urine or feces was negligible (< 5% of administered dose).Key words : potassium pentagamavunon-0, pentagamavunon-0, pharmacokinetics

Abstract

Observation of the antibiotics prescribing of out-patients was carried out by collecting retrospectively all of the prescription received in the period of October to December 2004. Percentage of antibiotics use were obtained from the data. Kinds of the most frequently prescribed antibiotics, percentage of antibiotics prescribed in patent/generic name, and percentage of antibiotics prescribed listed in the hospital formularium, also obtained fromthe data.The results shown that prescribed antibiotics reached 19,54%. The most frequently prescribed were beta lactam derivatives about 58,16%. The ones were prescribed in the patent name about 93,71%, whilst in generic name were 6,29%.Based on the prescribing indikator, the use of antibiotics were lower than 22,70%, which were lower than the lowest value of the WHO observation. That could be stated that was a rational drug use. There were also supported data that all prescribed antibiotics in the hospital formularium list.The prescribing in the generic name was very small.Key words : antibiotics prescribing, prescribing indicator, out-patien
